Appointments and Fees
Appointment Times:
Flexible assessment appointments are available six days a week, Sunday – Friday. In many cases we can offer same-day assessments, and are certain to find a time that fits your schedule. Assessment interviews take approximately 2 hours.
With the Covid-19 restrictions
we are REGISTERED with STATE OF MINNESOTA's DHS to
PROVIDE TELEHEALTH/VIRTUAL ASSESSMENTS
For a telehealth/virtual assessment you will need to be on a computer or appropriate cell-phone app in a private space for privacy and confidentiality during the assessment interview.
You will need to print the forms and scan them to fax or email to me.
Payment will be Payment by credit card.
Rates:
$275 for your comprehensive Substance Use Assessment
All major credit cards accepted
Insurance:
Assessments needed due to legal issues (DUI) are usually not be covered by insurance companies, hence we do not accept insurance.
Cancelation Policy:
If you fail to come to your appointment, or do not provide 24 hours notice you will be required to pay the cost for the meeting time.
